Each individual business traveler that Leck Travel Service works with is requested to fill out an Executive Traveler Profile.  This secure profile allows us to compile important information needed to book transportation and other accommodations for that particular person. 

This speeds up the process of arranging and confirming plans that will utilize all membership benefits a particular traveler may have.

Please download the interactive Executive Traveler Profile pdf file. If you do not have the updated software needed, you may wish to print the one page form, fill it out by hand and then fax it back to your Destination Specialist at fax number:  (817) 654-9529.

If you are able to download and complete the form on your computer, please save a version in your system for your future easy and fast reference. You may then email the form or, if printed, scan the document and email a scanned version to your designated travel agent. Please complete as much of the information that applies to you.
